How to Get Your DMV Record in Missouri

A DMV record is basically a written record of a driver’s history. DMV stands for the Department of Motor Vehicles but in the U.S. state of Missouri, this function is carried out by The Missouri Department of Revenue. MDR has many divisions, out of which the Motor Vehicle and Driver Licensing Division deals with the driver records. The department deals with the registry of motor vehicles and issues license plates and driver's licenses.
 
What is included in your Driver's Record?

There are two types of driver records that can be accessed by a person in Missouri. These are Driver Record with Personal Information and Driver Record without Personal Information. There is a slight difference between the procedures to request these two different types of documents.

A Driver Record without Personal Information has information regarding fines, speeding tickets, and other violations while a Driver Record With Personal information includes details like date of birth, sex, height, telephone number, driver license number, social security number, photograph, medical information, etc.

How can you access your Driver's record?

Requesting the Driver Record without Personal Information is very easy as compared to the Record with Personal Information. One can either directly go to any Missouri License Office and pay the fee or can send a written request including their name, date of birth and driver license number of the record holder to the Driver License Record Center along with the required fee if they want the record to be transferred by mail or fax. So, providing a mailing address or fax number is also necessary.

In case a person requests the Driver Record with Personal Information then they have to fill Form 4681 which can be downloaded from the official website of the agency (dor.mo.gov).


The Driver Record Request Form consists of five sections; in the first section, the requesting applicant has to fill their full name, date of birth, telephone number, Missouri Driver License or Social Security Number and complete mailing address. In the second section, one has to mark on the type of Driver Record(s) requested for and the fee has to be paid accordingly.

The third section asks for mailing and fax information. Here, you have to provide the alternate address if you wish the requested records to be sent somewhere other than to the record holder’s address.
 
In section four, a person needs to provide the card details if they are willing to pay by a credit card or a debit card. This section also has an undertaking for perjury penalties in which the record holder has to provide their signature and date. The last section includes the Notary information which gives a legal assurance of the character of the record holder or the requesting person, so it must be filled with caution.
 
Records can be obtained by walk-in, mail-in, or e-mail requests. If you want to request your own record in person then you can walk-in any Missouri License Office and submit your photo ID along with the correct fee. If you are applying for someone else’s record then you will have to submit the completed Form 4681 along with the correct fee.

If you want to request the record through fax or mail then submit the completed Form 4681 along with the correct fee to the Driver License Record Center.


The fee is $2.82 per record. A convenience fee will be charged for credit or debit card transactions which are clearly mentioned in the fourth section of Form 4681. In case of fax, an additional fee of $0.50 per page applies and if a person purchases the form at a Missouri License Office then an office processing fee of $2.00 will also apply.


Note: Only a person exempted under the Federal Driver's Privacy Protection Act (DPPA) is allowed to request for the Driver Record with Personal Information.
