How to Get a Birth Certificate in Oklahoma?

The Vital Records division of the health department of Oklahoma issues maintains and manages all public records including birth records of the people of the state. If you want to apply for Oklahoma birth certificate, you can do so in person, by mail or online, provided you fulfill the requirements for application.

Who is Eligible for a
State of Oklahoma Birth Certificate?

Oklahoma birth records are not open to public access. An individual is required by law to fulfill any of the following criteria to be eligible for a birth certificate:

  • Must be the subject of the birth record
  • Must be any of the following individuals acting on behalf of the subject:

o The parent whose name appears on the birth record

o Subject’s grandparent, spouse, step-parent, or adult grandchild or child. The individual must carry an authorization signed by the subject. An application must also include a copy of the identification of the subject in case he or she is 18 years old or above

o Adoptive parents or legal guardian

o A representative holding Power of Attorney

o The attorney representing the subject or his or her adoptive parents

o An individual authorized by a court order

o Genealogists or other individuals

In-Person Request

  • Download the birth certificate application available here.
  • Fill in all blanks. An incomplete form delays the process.
  • Visit the Vital Records office at any of the following addresses:

o If you are in Oklahoma City, then visit the state health department’s office located at the 10th street of 1000 Northeast

o If you are in McAlester, then visit the Pittsburg county office located at 1400 East College Ave

o If you are in Tulsa, then visit the James Goodwin Health Center located at 5051 S. 129th E. Avenue

You can visit the office from Monday thru Friday between 8.30 a.m. and 4.00.p.m., except on state holidays.

  • Carry one valid photo identification. Photo identifications are considered Primary Identification. Identifications that have been expired for 3 years are also accepted. See the list of all valid photo identifications here.

Submit a Secondary Identification if you do not possess Primary Identification. You need to submit 2 such identifications. See the complete list of accepted Secondary Identification here.

If you are an individual other than the subject or his or her parents, then you must carry additional documents as specified here.

  • Pay the applicable fee. The current fee is $15 per copy. You can pay using cash, money order or personal check. If you are using a credit card, you’ll be charged an extra $2 as portal fee and a 2.25% toward merchant fee.
  • Be prepared to wait for at least 1 hour to receive your certificate.

By Mail

  • Download the birth certificate application.
  • Complete the form.
  • Include a legible photocopy of your photo identification. See the list of acceptable photo identification.
  • Include the exact amount of fee either through money order or a check made payable to OSDH. The current fee is $15 per copy.
  • Mail the application along with the specified documents to the following address:

  • Wait for 6 weeks, which is the usual time for processing mail orders. You can request an Expedited Service, which takes only 5 business days. For this service, you need to submit a self-addressed envelope with the required fee. Call 271- 4040 for updated fee details.

Order Birth Certificate Oklahoma Online

  • Visit Oklahoma government-approved online portal for ordering a birth certificate.
  • Enter the required details as instructed on the portal.
  • Be prepared to submit valid photo identification. Sign and enter the date on the Identity Verification Form presented during the ordering process.

If you require additional documents as proof of entitlement, then you need to submit the same according to the instructions provided on the portal.

  • Make a payment using any major credit or debit card, including MasterCard®, American Express®, Visa® or Discover®. In addition to the standard fee of $15 for registration, you must pay $15 for online processing.
