How to Get a Birth Certificate in Vermont?

In the state of Vermont, it has been a legal requirement since 1857 for all towns to report all births. This information is collected by Vermont State’s Vital Records. Since information about births is stored in a public access portal, it is considered as public records. Birth records are important in order to monitor and review the health of the population. Apart from Vermont birth records, you may access information regarding marriages, deaths, civil unions, abortions, fetal deaths, dissolutions and divorces in the Vermont Vital Records System.

According to Vermont State law, all midwives and physicians must file a report of a child’s birth at the State Registrar’s office within five days. Once this report is registered into the Statewide Registration System, a Vermont birth certificate will be given upon request. Babies that are born without the presence of a professional must be reported by contacting Vital Records at 802-863-7275 or toll-free at 800-439-5008.

How to Get a Birth Certificate in Vermont?

In Vermont, there are four options available to order a certified copy of your birth certificate.

1. From the Town Clerk

A town clerk may issue you a certified copy of your birth certificate. To locate a town clerk, visit the Vermont Secretary of State website and select ‘Guide to Vermont’s Town Clerks’.

2. From the Health Department

Public access to birth certified birth certificates from January 1, 1909, onwards is available for a nominal fee from the Department of Health. To order your copy, visit Vermont Government Vital Records Request Service. Select ‘Start’. Complete each field listed under ‘Applicant’ and then select ‘Continue’. You will be requested to make an online payment for $12.00 USD for each certified copy of your birth certificate. This will take up to seven business days. For expedited order, the fee is $14.50 USD. Your state of Vermont birth certificate copy will be shipped to you overnight. All payments are to be made online.

3. Order Via Mail

To place an order by mail, you will be required to complete an application form. You may need to provide supplemental information. All payments are made via checks and money orders only. These must be made payable to the “Vermont Department of Health”. The fee to order birth certificate Vermont is $10 and an additional $10 per extra copy. Once you have completed the application form, send it with a photocopy of a government-issued identification and money order or check to the address printed on your form. Please do not send cash. 4. Order in Person

To order a certified copy of your birth certificate in person, complete the relevant application form and print it. Do not forget to read the Supplemental Instructions listed along with your application form. All payments must be made with a money order or check. These must be payable to the “Vermont Department of Health”. The cost of obtaining a certified copy of your birth certificate is $10. You may order additional copies at $10 per piece.
