1) Applying for the Certificate

You need to fill the application form completely and submit it along with proof of identification. Photo identification such as passport or driving license has to be produced. If not available two other documents like social security, bank statement, car registration, etc. need to be produced.

How to Get a Birth Certificate in Kansas

If you intend to apply through mail, you can download the form from http://www.kdheks.gov/vital/download/birth_fillable.pdf, fill it and attach proof of your identification. You can then mail the application to:

If you are applying online, you can do it through the website of VitalChek. You can visit https://www.vitalchek.com/order_main.aspx to apply online. The procedure is simple, all you need to do is fill the form and provide all your details. You need to attach a copy of the identification document with the application.

You can apply for a birth certificate by phone. To do this, you need to call (785) 296 3253 from Monday to Friday from 8 am to 4 pm. Follow the instructions on the phone to verify your identity. Once you provide all details over the phone, the application will be generated for you by the executive.

2) Pay the fee

Once the application is filled and the identification document is produced/enclosed, the next step is to pay the fee. The fee for obtaining a birth certificate in person is $15. For additional copies, you need to pay the same amount. You can pay by cash, check, or money order.

The fee while applying by mail is $15. This must be paid by enclosing a check or money order favoring Kansas Vital Statistics.

When you apply online, you need to pay $15 for the certificate with $11 for processing by credit card.

When ordering over the phone, the fee to be paid is $15. You need to pay an additional $11 towards call and processing charges. The fee has to be paid through credit card.


3) Get the Certificate

If you visit in person, you can get the certificate within 15 to 20 minutes, if there is no problem locating your records.

Mail applications are processed within 5 to 10 working days of receipt of an application, after which you can get the certificate by post.

If you apply online, you will get a mail from VitalChek indicating when you will get the certificate.

If you have applied through phone, you will receive the certificate within 3 to 5 business days after your application reaches.
