The Hawaii health department has records from 1951 to 2002. A fee is applicable to all death records requests. For the first copy, you will have to pay 10 dollars and four dollars for every subsequent copy. Mail – You can apply for death records by sending in your applications to this address. State Department of Health, Office of Health Status Monitoring, Issuance/Vital Statistics Section, P.O. Box 3378 Honolulu, HI 96801. You must enclose a copy of your photo ID such as State ID or driver’s license. A 2.50 dollar portal administration fee will be applicable.

Walk-in – You can also apply for death records in person by visiting Room 103 (1st floor) of the Health Department building, 1250 Punchbowl Street. The office is open from Monday to Friday between 7:45 a.m. and 2:30 p.m. The office is closed on all public holidays. You can either fill out a paper form or apply at the online kiosk at the office. You will have to submit a state-issued identification proof and the death records processing fee. Payments can be made in cash, money orders, or credit cards. Personal checks will not be accepted.

Online Death Records – You can also obtain Hawaii death records online. You will have to visit the Vital Records Website and follow the process to apply for Hawaii death records online. You will have to submit ID proof as well as proof to show you are eligible to get the death records of the deceased since these are not public records. You will have to pay 10 dollars for the first copy and 4 dollars each of you needs extra copies of the death certificate.
