Visit the vital records website

When you visit the website, you can place an order online to get a birth certificate. You can do this by ordering as a guest, by clicking on the option ‘Order as guest’. If you want to save your order details, you can create an account and log in to the website. Creating an account is easy, all you need to do is sign up by providing your e-mail address and a few details.

Place an order for the birth certificate

Once you enter the portal as a guest or by logging in, you can select birth certificate as the option to order. You then need to select the delivery method. You can choose to pick up the birth certificate yourself or have it sent by mail. If you choose to pick up the order, you need to visit the Health Department office at 1250, Punchbowl Street, Honolulu and drop into Room 103 to pick up the birth certificate yourself. If you choose to pick up, you need to mention if you would pick it up or send someone else.

You then need to specify the relationship you have with the person whose birth certificate you are requesting. If can be for self, mother, father, sibling, etc.

Provide your details

You need to fill the order form. You need to select the reason for requesting the birth certificate, which could be for school entry, passport, driving license, etc. You then need to enter your contact details.

Once you submit the details, you need to make the payment online. If you pick up the certificate yourself, you can pay by cash, else you need to pay online by credit card. The cost of the birth certificate is $12.50. This is for the first copy. For duplicate copies, you need to pay $6.50. While paying, it is important to ensure that the credit card must be in the name of the person for whom the birth certificate is ordered or in the name of the person making the application.

You need to upload a photocopy of the ID proof of the person making the request.

Issue of certificate

Once you submit the form and make the payment, the certificate will be sent to you within 8 weeks. You can track the order online on the website. You will know when the certificate is ready. If you want to collect the certificate in person, you can visit the office and collect the certificate after showing a copy of the ID proof.
