How to Get a Birth Certificate in Wisconsin?

The office hours are Monday to Friday from 8 a.m. to 4:30 p.m. On Saturdays, the office is open from 1 a.m. till 6 p.m. It will take up to 20 minutes to process your in-person request. The charge for a single certified copy is $20 and $3 for additional copies. Payment via debit and credit cards are not accepted.

How to Get a Birth Certificate in Wisconsin

It is important to carefully read the instructions on the application. If you are not an immediate relative of the person listed on the birth certificate, you may obtain a certified birth certificate copy under the following conditions:

1) You must carry a notarized and written authorization which clearly states your relationship with the authorizing person whose name appears on the birth certificate; or
