How to Get a Birth Certificate in Louisiana?
Louisiana’s Office of Vital Records registers public records including birth records of only those births that took place in the state. Louisiana does not permit unrestricted access to vital records including birth records. This means an individual applying for a state of Louisiana birth certificate must submit a filled-in application form as well as valid identification and applicable fee, to receive the aforementioned document.
Who is Eligible to Apply for a Louisiana Birth Certificate?
- The birth record subject, meaning the individual in whose name the birth record is registered.
- Any of the following family members of the record subject is eligible to apply for a birth certificate:
o Either parent.
o A sibling
o An adult child or adult grandchild
o The current spouse
o An adult grandchild, or grandparent
- An attorney acting as a representative for the record subject or for any of the subject’s family members listed above.
The attorney must submit a contract signed by the parties involved, authorizing his or her representation.
- If an individual applying for a birth certificate is not any of the above, then he or she must submit a custody judgment. Custody papers that are notarized or custody mandates that are provisional in nature are not accepted.
What Identification can an Applicant Submit?
An applicant can submit 1 identification copy known as the primary document or 2 identification copies known as the secondary documents. Expired documents are not valid identification.
The list of valid primary documents includes:
- Driver’s license
- A photo identification card issued by the U.S. Military.
- Any photo identification issued by the state.
- Certificate of citizenship issued by the U.S. Government.
- Any other photo identification as specified here.
The list of valid secondary documents includes:
- Social security card

How to Get a Birth Certificate in Louisiana?
1) Apply for Copies of Louisiana Birth Records in Person
- Visit the central office of Vital Records located at Suite 400 on 1450 Poydras Street in New Orleans. The office offers a walk-in service from 8:00 A.M. to 3:30 P.M. The office closes for state holidays.
- Carry valid photo identification or secondary documents as applicable.

- Visit your local Clerk of Court.
- Submit completed application and required documents.
- Make the required payment. You have to pay extra in addition to the standard certificate fee of $15.
- Visit a birth certificate issuance self-service kiosk. The operating time of these kiosks is between 8:00 A.M. and 4:30 P.M. Kiosks are unavailable on state holidays.
- Complete identity verification process.
- Make a payment for the certificate, security, and shipping.
- Receive the certificate either at the Vital Records office or through expedited or USPS delivery.
2) Apply for a Louisiana Birth Certificate by Mail
- Print the application and complete it.
- Enclose valid photo identification or secondary documents as applicable.

What Information Appears on a Louisiana Birth Certificate?
There are two types of Louisiana birth records. Short-form birth certificates, also called birth cards, include the name at birth, date of birth, parish of birth, father's initials, mother's last name and first initial, file date and issue date. They cost $9.
